Switch from being a Chemo Angel to a Card Angel

We understand that finances and circumstances can necessitate a change from Chemo Angel to Card Angel. However, every patient is assigned a Chemo Angel and a Card Angel when they join us. Your patient already has a Card Angel and will need to be assigned a new Chemo Angel. Please indicate on the form below whether you would like to continue as a second Card Angel to your patient, or relinquish your current assignment and apply for a new Card Angel assignment.

For Angels who did not send weekly If you sent less than once a week, can you please assure us that you will be able to send at least once a week to your next assignment? Please consider this carefully. If you found sending once a week difficult with your last recipient, perhaps Chemo Angels is not the right volunteering venue for you.
